For quite some time now, I have had worrying vaginal discharge that has refused to go. I am embarrassed and distressed and wonder what can be the cause of this? Can you please explain the causes of the condition

Dear Madam,
The best way to find out what is the cause of a woman's discharge is to do a test called HVS-- High Vaginal Swab-- a test done by the doctor to discover the cause of a woman's discharge or infection
Note these ten points about vaginal discharge and you will have rest of mind
1. All adult women have a Normal Vaginal Discharge that is white like coconut or akamu; does not smell and does not itch. It is also, not plenty.
2. This normal discharge can also be a little abnormal in one or two situations-- normal discharge that has become too plentiful. The first is during ovulation-- this is referred to as ovulation mucus.
3. There is also what is referred to as Leucorrhea in pregnancy -- the pregnant woman secretes and discharges normal vaginal secretion more than the non pregnant. This is not disease in as much as all of us agree that pregnancy is not a disease!!!
4.Of course, after sex, when the penis has gained entry into the vagina, there is an artificial increase in the woman's discharge --it is nothing but the excess seminal fluid flowing out of the vagina; okay? Nothing to worry about.
5. Candidiasis, Moniliasis or Yeast infection are synonyms for the same condition that is recognized as the single most common cause of female genital discharge as well as itching among human females. It needs treatment. It is not normal. The discharge is whitish or creamy, itchy seriously and plentiful.
6. Trichomoniasis is another cause of vaginal discharge. Trichomoniasis is itchy in the extreme. It affects the male partner also; who unless he is treated concomitantly (at the same time) will promptly pass it back to the female in an eternal ping-pong like game which the inexperienced doctor cannot stop. The disease can be transmitted via toilet seats or swimming pools. The discharge here is not white, is seriously smelly (at times like fish) and can have funny colours-- green or yellow.
7. Gardnerellosis or Bacterial Vaginosis is the name of a third affliction that can cause abnormal discharge from the vagina whose treatment is the same as Trichomoniasis and is named after a certain Dr Gardner who discovered it.
8. Vaginal discharge can also be increased and abnormal -- but not itchy in Chlamydia-- a Sexually Transmitted Disease where treatment must also include both partners -- and which can cause infertility. Chlamydia infection will almost always also affect urination –with pain, burning or discomfort, frequency or other troubles in addition to the discharge. It is important to note, also, that in Chlamydia infections it can happen that there may simply be no symptom at all, in some cases.
9. Gonorrhea causes a cervicitis -- inflammation of the cervix -- that gives a vaginal discharge too and the only way to find out if it is gonorrhea is to do the test I mentioned above-- called the HVS--High Vaginal Swab with m,c,s-- microscopy, culture and sensitivity.
10. The only other time a woman may have some bad discharge is when there is a foreign body inserted into her vagina--forgotten menstrual stuff or tampon usually or some other foreign body. This is why any complaint of Vaginal Discharge must be thoroughly investigated. There is no way a lady having discharge can be properly treated without examining her vaginally; no way. And, like I said, there must be an HVS taken to find out the exact type of germ causing the infection; so that appropriate measures to save the woman's reproductive health and fertility can be taken. Sir, I was suffering from stomach ache I now decided to go and see a doctor he now advised me to go for scanning the result states that my right kidney has parenchymal inflammatory changes, with echo sonic texture, loss of cortiomedullary differentiation, no calculi or hydronephrosis. My uterus is normal and anteverted, empty cavity, no gestational sac seen, my ovary is normal, no cysts. This is what was written at the end: “RT.PYELONEPHRITIS; PID”. Sir, help me explain all these. The doctor only said I have an infected kidney. Thank you

You clearly understand the words "normal", 'no" and "empty". Pyelonephritis is just what the doctor called it -- it's infection of the kidney and needs to be tended to properly; so go see a very good doctor. If neglected, pyelonephritis can become a cause of Acute Renal Failure which if allowed to turn Chronic is usually irreversible and terminal, ending ultimately in death. PID is Pelvic Inflammatory Disease and means your tubes and maybe other pelvic organs are infected. PID can cause decreased fertility, so you also need to get it treated fast -- and by the professionals trained to do the job -- doctor/gynecologist, okay? Both these conditions are usually caused by ordinary, non-sexually transmitted bacterial germs found in the stool which arrive in the vagina and bladder from the backside often as a result of sex ( --? maybe from behind?) -- so prevention would mean greater cleanliness in the woman's cleaning up --after stool, sex, urination, menses or whatever --from behind back and not forward -- and then if from behind intimacy has happened, proper cleaning of the genitals before penetration."Honeymoon cystitis" (infection of the bladder) during honeymooning is a well known precursor to pyelonephritis. Esherichia coli, Coliform bacilli and Enterococci faecalis as well as Staphylococci, Klebsiella and Proteus are usual microbial culprits and must be flushed out with proper antibiotics to which they are suitably sensitive in the correct dosages for them correct period of time. Pain, discomfort, pain in the lowermost belly--or pelvis-- waist or back pain are all features of both these diseases and in the acute stage, of course, fever and chills. With PID there may be no frequency, pepperiness or other abnormality with urination which are normal in pyelonephritis but there may be abnormal vaginal discharge and pain with menstruation that was not present before or not as intense. Women get this kidney problem 3 or 4 more times often than men because of the relative shortness of their urethra or urine pipe. Summary? See a good doctor/gynecologist and do that fast.

Dear Dr. Please I urgently need your help.
I am a married woman for the past 3 years now
without issue. I have never been pregnant before. My
husband and I had gone through series of tests which proved
nothing. We are asked to just relax and wait on
God. The problem is that I usually experience sperm
gushing out of my private part after sex even though I did not stand
up on or from the bed. This usually happens whenever my husband
withdraws. What can I do? Doctor said everything
cannot come out, but I am sure, it is everything
that is coming out. What can I do, even when I lie and sleep there for hours
or support my waist with pillows the result is the same
all the time.

Many of our readers will remember that I always talk
about approaching issues of infertility-- or any serious health problems for that matter
--especially the ones that defy treatment or cure by using the three-pronged holistic approach – the physical; the mental and the spiritual. Your type
of case, madam, buttresses the need for such an
approach. I still am waiting for the scientific
explanation for what you just described. The vagina
is a potential space--just like a pocket does not open unless you put something in it, so also does the vagina remain closed until the penis goes in.
There is no more efficient "safe" than one like the
"pocket" that the vagina constitutes. Whatever you
put in it, it closes in on it and grips it --
that's the natural thing. Now, when what you put in is a
viscid, sticky liquid like we all know semen is,
it's even better-- much less likely that the stuff
is going to fall off even when you stand up and
turnthe "bottle" or barrel of the vagina upside
down. But listen to yourself and a million other
women in this dilemma -- You do not even stand up
from the bed-- you actually, even put pillows
under the buttocks to make the bottle gain a sharper
incline and yet the whole thing flows out
immediately after sex!!! Now, the big question-- if
you poured water into a standing bottle, will the
liquid flow out if the bottle is still standing on
its base? Or even if you inclined the bottle and
made it stand on the edge of its base but almost
perpendicular -- could liquid flow out? Excuse me,
the answer is a resounding No. No; no, no! That is
why infertility is a matter beyond just the
physical
tests and procedures and ovulation etc. Or even
hi-falutin gimmicks like ICSI and IVF. It is a
matter of body, soul or mind and spirit. Or why
would liquid flow out of a bottle that's standing
on
the edge of its base? That is not natural. It's
not
physical -- you will agree with me. It's not
mental
either (though there are mental dimensions to
infertility management). It definitely is
spiritual!! The truth is -- there are forces
beyond
the natural that have refused you a pregnancy all
this time and are efficiently pouring out your
husband's semen from your vagina immediately after
ejaculation, so that -- you guessed it-- you will
never get pregnant. That's it -- and nothing else.
Remedy? Go it the way I always advocate --- take
care of all areas --body, mind and spirit.
Spiritual simply means prayer. After all, we care,

For the past four years I have been trying to get married with no success

but a spiritual consultant. Although I'd say it would be advisable to see

as visual hallucinations--seeing things others do not see-- and that is a
major feature of schizophrenia(not being right upstairs).
But maybe you deliberately sent to us because you know that apart
from being a medical doctor we are also into the spiritual dimension.
The problem should resolve or at lest reduce considerably if you got
married; don't you think?
The possession aspect you mention may or may not be the root of
your problem. See a psychiatrist as well as a deliverance
minister. Or if you cannot find one, do it your self by praying
(and maybe fasting) because as you may know prayer changes
things --even science is beginning to discover this.

Malaria can present the way you describe.
Use the correct treatment for it-- too many times people are dealing with resistant malaria and the longer it stays, the more anemic --not enough blood --the patient gets; explaining why you are dizzy and stagger. You see the malaria parasites destroy blood cells that's how anemia gets in. ACT = Artemisinin- based Combination Therapy is the correct way to treat malaria, these days. It must include, Artemisinin-- the new anti-malarial from China; and whatever anti-malarial you used to use before now. Get a good doctor/internist to look you up and treat you for not only malaria but also for anemia and you should get and feel fully well

Please can you help me? I have all these symptoms and I am scared to go see a doctor am not even sure we have a competent one where I live. I started having painful menstruation last year ending then the waist pains began the headaches come and go but am not able to jump around and play because my head aches when I do. You have three problems --very likely. One is PID. Pelvic Inflammatory Disease can afflict women even if they never had sex before; once a woman who did not have menstrual pains previously, begins to have them, it is safe to assume she now has PID; simple. Waist pains and backache make me believe PID is actually what you have. Untreated PID can help cause Infertility. Then you likely have typhoid with your come and go headaches. Very likely, the headaches will be affecting your eyes and forehead majorly. You also have another problem common among women but not an STD, and that is Candidiasis. That's very likely the cause of your on and off itching genitals. Our society needs more people of your type.Without wasting much time, I have been having this problem.I urinate a lot, and I have been experiencing serious body hot internally.And I'm very slim.I have been drinking a lot of water but I still pass a lot urine especially when I take water. Please sir, this is giving me sleepless nights. Please, advise me on what to do, because, I don't want to continue loosing weight. Diabetes Mellitus is the disease that readily comes to mind when you talk of your problems. It is easy to confirm or deny whether or not it is the cause of your symptoms. Very easy. Just ask a doctor to send you for these 3 tests: a)Urine Analysis; b)Random Blood Glucose ;c) Glucose Tolerance test. The results will tell if you have diabetes or not. Type 1(childhood onset) diabetes classically shows itself with 3 features a)Polydipsia-- too much water drinking; b)Polyuria -- too much urination; and c)Polyphagia with weight loss-- eating too much but not gaining weight -- in fact, actually losing weight! You can see that you almost fit this description. With Type 2 or maturity(adult) onset diabetes; there is usual weight--gain (not weight loss) as a predisposing factor -- not as a consequence. Of course there is the ubiquitous polyuria -- too much urination and of course, over much thirst and water-drinking. Why cannot I prescribe drugs for you to gain weight without seeing you and examining you and investigating you properly? Because there are many diseases that cause weight-loss. If you do have Type 1 Diabetes, you could just die-- yes --actually just die while taking some general weight-gain medicines that I could write for you. Die from the diabetes. You see, Type 1 diabetes kills without the use of insulin because the organ called the pancreas that produces insulin in the human being is sick --very much so -- in the diabetic; and so, does not produce any insulin again.
