Dear Doc,

I am the one that called you yesterday, I am a 33yr old man from Enugu State. I have a serious problem which is trying to ruin my life. I engaged in a relationship which resulted in disease and right now I am dying under some terrible pain. When I urinate I always pass some milky fluid. I have a swollen scrotum, I was diagnosed by a doctor who said that I  had a hernia. I went through surgical operation but to no avail. I have taken my problem to so many hospitals without any better result.

These are the symptoms: milky fluid coming out of my penis when I urinate, swollen scrotum (testis), weak sexual emotion, pain in the swollen scrotum (the right one). I suffer the pain most when I have a strong erection and sometimes migraine headache with it. My lady has loved and followed me and our relationship has resulted into marriage; and on the bed, presently I am now faced with these sexual problems--

1. I am unable to go more than one round , and release my load in a second.

2. After releasing I could not go again until the next day.

3.The most worrisome part  is that I cannot have or maintain a good and strong erection, my manhood always as soft as foam.

4. My early morning erection is no longer there.

5.During ejaculation my semen used to shoot but now it oozes out.

Please I am always ashamed of myself whenever she calls for sex. I am thinking of losing my life than live and seeing myself in this unbearable condition. My wife has tried to bear with me but right now, she is threatening to leave me if there is no solution to it. I also pity her for what she has being undergoing for so long.

I was subjected to a lab. test and below are the result. *Motility rate 10% Actively motile 30%. *Sperm count 5x1000000 cells. *Viscosity.. slightly viscous. *Pus cells... 1-3hpf ph 7.5. *Morphology..25% abnormal cells. The test also showed that I have a STAPH AUREUS infection. Please I still count on you for help. Sir, I want you introduce me to professional doctor in this area or the kind of drug that will eliminate the pain and restore back my potency. You have a Chronic Prostatitis --any male that discharges in the absence of sex has it--and your case shows how pathetic ignorance (of even medical practitioners) can be. Sexual activity tend to be vigorous, wild and unrestrained among young men. They catch a bug and infect their prostate -- an internal organ in the male that can be called a man's womb. Prostatitis sets in and can be hard to get rid of.

a)Sexual inadequacy(SXI) or erectile dysfunction

In a nutshell, understand that Staph --or any other germ (especially E.Coli, Kelbsiella, Proteus; Streptococcus etc --  some of them STD's and, some not so) can invade a male's system, infect his prostate; give him a CPR and then cause SXI secondary to the CPR. My Complaint About African Doctors

Dear Dr Bibbi,
Your write-up on the subject of Organ in Bondage/Sexual Inadequacy in the Daily Sun recently, refers. It made an interesting reading. I am worried that some Africans Medical Doctors do not take the spiritual aspect of ill health into consideration while managing cases. As a result when you make your complains they tell you that it is psychological especially when sexual problems are concerned. It is sad indeed. I have a similar problem to that of the writer of  that piece. I got married in Dec. 2013 and since then it has been one form of spiritual attack and the other. Oppressed by witches while sleeping, finger prints on my body, multiple sex dreams with unknown partners, had two fatal motor accidents, and now I find it difficult making love to my wife because my organ will not rise or become erect!!! Since we got married we have believed God for the fruit of the womb but surprisingly I find it most difficult to make love to her during her ovulation.
Once I or my wife have this sex dream it becomes  difficult for me to get an erection. The size of my organ becomes  so weak and very smallish. Just as written by the complainant in his letter I have to beg and rub my organ before I can manage a weak erection sometimes. All these are the opposite of my former self before I got married. 
 My type of job take me away from home for a week and gets me off duty for a week. If I am alone in my station I WAKE UP almost every night to see my organ strongly erected usually about 1.30am -3.20am.
I am a born again Christian so all we have being  doing is to get into stubborn and aggressive prayers. But the more we pray the more the problem gets tougher. The joy that greeted our marriage is almost disappearing.We  are weeping, bruised and heartbroken.
 We consulted a Medical Dr (Gynaecologist) who put me on PROVIRON, to solve the problem and, yes, the first three days of the ingestion of the drugs it worked, but  thereafter it was as if the drug was working the opposite of what it was supposed to do. (I have taken the drug for about 3months now.) How can orthodox medicine explain that?  I must tell you that some persons ( my former girlfriend who is Genotype AS  same with me, as well as the guy my wife had relationship before I met her) put obstacles on our ways to stop our wedding but God saw us through. They even threatened that if I married  my wife that our marriage will not last.  Certainly out of ignorance I did not take their threats serious, but now I know better. 
 Doctor, it has been a difficult time for me and my wife. Our problem is no doubt purely spiritual and we have taken it like that. We have taken our case to the supreme court of God Almighty. We believe God is able  to deliver us and give us victory in Jesus Name.  On this basis, it is painful when our fellow Africans
 Doctors do not accept that some ill health are spiritual; rather they will say that such sexual problem is the state of the mind. Those finger prints on the body on waking up from sleep, those strong erections in the midnight when alone, etc are they also psychological?

Let me help you by making some six pertinent points. 1)Doctors, African or no;t are doctors, not priests, pastors, clerics or shamans. They do not have any training or exposure in school to the things you are talking about, so, they cannot and do not handle such matters. 2)Doctors take training in physical sciences and psychological sciences; full stop. Nobody teaches them about he spiritual and it is not the fault of anyone. 3)The scientific basis or way of thinking that is the root of their training does not recognize anything THAT CANNOT BE SEEN or PROVEN PHYSICALLY. 4)You will agree that the kind of understanding you say is lacking is more available in church or seminary, Bible, Koranic or spiritual school. 5)In point of fact, because the average African understands these things without any doubt, African doctors tend to be more understanding than their Western counterparts in these cases. 6)For you to get not only a sympathetic hearing, but help to get rid of these your troubles, you need the rare professional who combines an understanding and proficiency in these two worlds that seem anti-thetical to each other-- the physical and the spiritual. Dear Doctor,

I happened to come across your column at a friend's place with whom I am staying presently and I want to say I like the way you advice and re-assure those who write you with their problems.This has encouraged me to write to you. I would like to know if it is possible for someone like me whose parents are not diabetic to have diabetes .Is it possible for a young boy like me who is twenty-one years to have diabetes just like that? What are the symptoms of diabetes? Does it make the male organ small and weak? Why I am asking this is because sometime ago I noticed that after I urinate, some whitish liquid comes out and it draws like okro though it's not quite as thick. It is clear and whitish and doesn't smell badly or cause pain . I thought I had STD at first but I was surprised because I have never slept with any woman before and I don’t masturbate. Last month I summoned courage and went to the hospital and to my greatest surprise I was told that it was semen escaping from my reproductive organs.Now one of my friends has told me that this is a sign of diabetes. Is it true? I am afraid because I know I take a lot of sweet things. Another one of my friends told me that this condition is caused by smallness of my organ. He further advised me to start having sex otherwise it would start growing smaller. Dear doctor, how do I stop this problem? It has been embarrassing me and it sometimes stains my trousers . Do I go for a diabetes test? Likewise is it really true that lack of sex on reaching maturity causes the male organ to remain small? How do I enlarge my organ? Is it through starting to have sex frequently?I thought of going to one of these places advertised frequently but I was held back because I know that some of them are quacks and I don’t know if such enlargement would leave me impotent .

Diabetes does not of necessity pass from parents to children-- its inheritance pattern is not so direct--that is even when parents have the disease. Yes diabetes runs in families but there are environmental determinants that can trigger its manifestation even when the genetic disposition is not there. So diabetes can just arise out of "nowhere"--yes. Diabetes does not the cause any of the things you have(although it can cause low libido--disinterest in sex as well as erectile dysfunction or impotence). You say you've not had sex before but what about necking and other such things including genital or oral contact? They are significant in these cases, you know. Do your test but let a doctor order it and it should prove that you don’t have diabetes. Diabetes is not always inherited. It can arise from nowhere. It's major symptoms are frequency of urination, overmuch thirst, tingling sensations in the feet and hands, frequent infections like boils, furuncles and carbuncles, slow healing ulcers, or cuts or wounds, blurred vision, always tired or just no symptom at all.For those with no symptom like you but who want to find out if they could have or be prone to the disease the laboratory may find they have what we call an impaired glucose tolerance when and if they do the test of their glucose tolerance. This your okro like discharge is a sign; not of diabetes but of chronic prostatitis –a disease that is usually a complication; not of an STD(it can be) but of UTI (urinary tract infection) which any one can have whether they sex or not. The genitals, you see, perform three functions: recreation, urination and procreation (reproduction). Now, chronic prostatitis is a very insidious disease and because the prostate is an organ through which all these three functions of the genitals pass, it could cause problems in all the three-- that is the cause of your problem and not diabetes. Anyone who, as a male, discharges any stuff without sexual arousal has chronic prostatitis-full stop. Most young men your age feel that they must have a horse size organ to be men. Not so. It is not quantity in this matter but quality that women consciously or unconsciously look for. If you have a king size organ and lack technique your sex would not be able to satisfy a woman-- never forget that.Also never forget that it is indiscriminate sex that causes problems(HIV,STD,AIDS ), not diabetes. Again, remember that diabetes does not cause these problems that you ask about. Your friend will push you into indiscriminate sex and that's where the trouble will come from; Okay? So beware.Small organs are not the cause of discharge, diabetes or anything; Okay? Don't buy that notion.It is un-educated; illiterate. Full stop.Diabetes is a disease caused by the body's inability to handle sugar /carbohydrates properly because insulin is deficient or not working. It does not mean that sweet things cause diabetes--else ask the people of Western cultures who would have been suffering an epidemic of diabetes on account of their liking for sweets at almost every meal--dessert, iced cream, jelly, syrup etc. A correct, balanced and nutritious diet as well as exercise and weight control are the things that help prevent diabetes.

I'm a 24yr old guy who is indulged in self-discovery and who is looking to self-actualization and self-fulfillment. How glad I am this day, sharing with you a problem that has -for some years now- been weighing me down. a problem that instills in me the feeling of faltering while partaking in an activity, of not doing what I ought to do, and the unwarranted feeling of scrutiny and social insecurity whenever I'm in a public place. it is a secret enigma that I've been living with -hiding it within the depths of my mind- for far too long. Pouring out my gravely over burdened heart to one whom I feel would understand and bear with me -and possibly help alleviate or eradicate- the ceaseless pains, sorrows and worries which I feel deep inside, I believe, is one great step I'd be taking toward recovery from my burdens. I've always taken it to be a personal affair; if it wasn't a problem I see as being so humiliating, I'd have referred to it as "my dear." This is the very first time I'd discussing -with anybody, not even my parents or siblings - the awful experiences in my social life. Quite strange and incredible, isn't it?
