Dear Doctor,
I have a problem that is multi-pronged. It all started last year when I was hospitalized due to bronchial pneumonia. I was placed on a 6-hourly injection for 4days. I recovered to find out I no longer had a job. This took its toll on me among other disappointments. To my problem proper; I tried making love to my fiance a month after my illness, but discovered to my chagrin that anytime I attempted wearing a condom, I lost my erection. All subsequent moves to get it back, results in involuntary emission. Initially, I thought it was a result of the illness that I need sufficient time to recuperate properly before embarking on something like that. I waited for another month before trying again and the same thing happened. I tried again the third time and it became worse, while putting on the condom, I released and all attempts to get back an erection proved futile. By this time my fiance has already lost patience with me and resulted in the subsequent breakup of the relationship. For your information, in the first month, my fiance showed me a lab result that indicates that she has scanty growth of staphylococcus and candidiasis. At first I was worried, but I remembered that I just came out of the hospital where a lot test was carried before my subsequent treatment. I felt it should have been detected if I had it. So, since the doctor didn't mention anything like that, I just shrugged it off. I don’t know if this is related to the problem at hand. Though I thought it was more of psychological than physical since I was kind of depressed. I have already encountered worse emotional problem before and my performance was not in any way affected, so I was really disappointed that it is happening to me now, especially when I’ve promised never to make love without a condom until I get married. I tried sorting things out with my fiancĂ© but she seems to be so sure that the episode will repeat itself and since the last day she no longer loves me. I am really worried about this development, please advise me as it is affecting me emotionally, and has ruined my love life, even worse than my unemployed predicament. When a person has disease of the kidney the manifestations are many but let’s look at maybe a dozen or half dozen signs and symptoms. But first o underscore the importance of your question
a) Kidney disease has been on an alarming increase in Nigeria in recent times that doctors and nephrologists have cried out at the sheer number of people it is affecting. b)Kidney diseases usually end in death --Chronic Renal (kidney) failure is irreversible--once a person has it, they really cannot be helped anymore--they will usually die --unless of course, new kidneys are obtained by transplantation surgery and the person gets a new lease of life. c)Kidney transplants can be very risky--the transplant may fail; the costs are prohibitive and the surgery and/or procurement of donor organs not easy. So it is really just the best for one to make sure they are not affected by this problem.
First is that there may be no manifestation--especially when the disease is still very early .
But there is usually some pain in the flanks or back--this is because the kidneys are to be found at that location--that is the flanks-- there will be edema--swelling of first, the legs but later all parts of the body from water retention and electrolyte imbalance. You see the kidney's job is to purify the blood from poisons in the system--it filters these poisons from the blood and then excretes them as urine. So the kidneys make urine.
Now when the kidney is malfunctioning thro' disease, of course the sufferer cannot make the amount of urine he used to make before or any at all. We call this oliguria or anuria. All that water that should have gone off with the urine now accumulates in the body causing edema.
The patient will also be tired, from anemia or lack of enough blood--the kidney produces Erythropoietin which triggers the manufacture of Red Blood Cells. Kidney disease therefore will cause lack of enough blood--anaemia.
The kidneys produce a substance called angoiotensin that helps to control blood pressure making it normal. A malfunctioning kidney therefore causes hypertension--nothing to control hypertension anymore.
You will agree that a weak, tired and exhausted, sick person will not likely be interested in sex. In kidney disease, there is loss of libido and impotence; there will be all sorts of menstrual problems and irregularities --maybe even amenorrhea--total loss of menses
Urea is the main poison that the kidney filters from the blood. High levels of this poison, urea, causes hiccups, nausea, vomiting and itching.
These are some of what you can expect when someone has kidney disease.
As for your second question all organs in the body that are double-- left and right-- can continue working at over 75% efficacy without the counterpart organ. Can a one -eyed man see? Of course he can
So one testes is enough the functioning one works harder, as it were, to compensate for the deficiency

I read a Daily Sun newspaper and saw so many problems put up to you which you gave a solution of how to solve; that was why I intended to present my case to you .This has been bothering me for long, and has been a very big problem to me which nobody will not able to observe, looking at me I'm healthy but inside I know the kind of problem I have. I've been to so many hospitals and have taken a lot of antibiotics, the lab always prescribe that I have typhoid and malaria parasite, and also stomach problem which the doctor said that I had excess gas syndrome. It has been 2 years and some months since this sickness started; I have never been myself again, and never believe that typhoid will ever last this way. If your stomach is acting up with excess gas-- and the doctor called it a syndrome then maybe we are dealing with something that is difficult to delineate. A syndrome is always so called because of this --it is difficult to characterize or understand; but it is a recognized group of symptoms or challenges to health or well-being. Typhoid certainly can be the culprit. Also, chronic indigestion can do the same thing --just as the poorly understood Irritable Bowel Syndrome (IBS). IBS means your bowel doesn’t work the right way; it can cause cramping, bloating, gas, diarrhea, and constipation, but doesn’t damage the bowel or lead to other health problems. The doctor, who will diagnose IBS based on your symptoms, may need to have medical tests done to rule out other health problems. Stress doesn’t cause IBS, but it can make your symptoms worse. Fatty foods, milk products, chocolate, alcohol, and caffeinated and carbonated drinks can trigger symptoms.
Eating foods with fiber like vegetables and fruits like pawpaw, pine apple, apple, cabbage, carrots, bananas, and eating small meals (up to 5 or 6) throughout the day may reduce symptoms. Drinking lots of water helps too. There is no medical cure for IBS but treatment for IBS may include medicine, stress relief, and changes in eating habits. You'll certainly need to see a good doctor face to face and have him investigate the IBS. Just seeing malaria parasite in the blood of anyone living in Nigeria's holoendemic area is nothing to write home about—it’s of no consequence especially if there are no accompanying symptoms of malaria

I want to use this medium to thank you for the work you have been doing to many Nigeria and worldwide. More grease to your elbow, sir. I want to share my own problem with you and I know you can help me as well. Sir, it all started in 2005, when I was schooling. I discovered that I have been experiencing pain whenever I go to urinate. It was not all that serious because it will come and go. Not until 2008, I discover that the pain is still trigger me and by then I went to a private hospital here in Abuja and did a urine and HIV test and I was told that I have Staphylococcus and my HIV status is negative because then I do have internal heat, some time blood in my urine, body weakness hotness in my body and so on. Because of the money I was told to pay before treatment commenced which I couldn't pay I stopped the treatment then.
Sir, I was living with the pain in my body till December, 2010 and I went back to the same hospital and did another test both HIV and the result still show that I have staph and my HIV status still remain negative but sir, am afraid.
Sir, the main reason why I am writing to you this note is that, now I am having chest pain about one month plus; I did not hit my chest on anything. Although, the urinating pain is not often the reason I said it is that, sometimes I will not feel the pain like two week but any week it will come I will not feel better.
Sir, I am afraid I may have liver problem or what could be the problem that my chest is paining me?
Please give me some health tip and as well advice me on what to do because am tired of living with this pain.
Sir, tell me what is wrong with me? Also, the money I was as pay in the hospital I went to is N120,000.00 for three months treatment but I don't have such money to be sincere with you.

Garri Troubles?

Thanks for your advice on sundry issues by different people. I want to know what is responsible for somebody to be having swollen eyes after taken garri. What is effect on health, the repercussion and how can it be rectified? For now abstinence from garri is considered but for how long? Please help.

Although I have never heard of this before, there must be something in the garri the sufferer is re-acting to, that is all-- it must be some allergic reaction and the sensible thing to do like you have said -- is to abstain from garri until you get to the root of the allergy with the help of an allergy doctor.Allergy specialist? Ask to see a dermatologist at your General or Specialist hospital.

The answer you seek depends on so many factors that no one can really say. That's the bad news. Simply let him watch out over the next 6 weeks or so for any untoward signs and get himself promptly treated for any suspected trouble. This means he should assume he got infected, pure and simple --until he can prove otherwise, okay? Next time -- (and there should be none; he should go and marry!!) -- let him take the precaution of wearing two condoms!!! What' the good news?  I've been having this problem of internal heat and wormlike movement after having sex with a girl 13years ego. Though I didn't go for treatment after noticing some changes in my body, I thought it was normal as I was growing up. I was living with these symptoms for 3years before deciding to go to hospital. They ran a test and discovered it was heavy growth Staph Aureus and I was placed on Cipro-oflaxacin 500mg for 1month. I was responding to treatment but after treatment, wormlike movement, body itching and internal heat refused to go away. After then I rejected going back to hospital and started taking antibiotics of different kinds on my own. Now I'm 31 years old and still living with these symptoms of internal heat, wormlike movement around my body and body itching. What do you advice I do? No antibiotic seems to be working on me anymore.

Very simply, when kids below ten struggle with enuresis and bedwetting, we attribute it to age and give them time to sort it out. It is something to do with their physical growth and development of their urinary system. Again, when adolescents in their teen years continue to bed-wet, we all should understand that there are underlying factors of psychology at play –they are struggling with some conflict or the other, maybe at school or with their peers or with whatever and we help them with deep psychological counseling and psychotherapy.
I have discovered that when adults (18 and above) like you have the problem of bed-wetting - and this problem, strange as it is, is not uncommon, the problem lies beyond the body or the physical; it lies beyond the mind or the mental; it is somewhere in the spiritual. The way to tackle such challenges is to do the physical necessary physically and psychologically but to add a heavy dose of prayer to the therapy. I have been reading your column on the newspaper and it is indeed very interesting the way you answer and also solve some people problems. I am 32 year of age. I have my own health problem that has been disturbing me for about four years now but could not get solution to it. Sometime in 2009 I fall ill the type of the sickness is that, I started having some difficulty in sleeping which lasted for some weeks. My head became so heavy my concentration became very poor, my body trembles also all my muscles seems to contract involuntarily especially my legs. I could not rest generally in fact it is almost impossible for me to explain exactly how I feel.
I went to see a doctor in a general hospital in Jos. After conducting some test he came up with a result. The result he says was anxiety neurosis. Which he gave me some drugs to take and see him in two weeks actually when I used the drugs I felt better. When I graduated from school, I was posted to Abuja in year 2010 since then I have been suffering from this problem on and off. I went to hospitals any time I have this problem, but I am sorry to say most of the doctors do not listen to my complaints they only quickly write drug for me to go and buy. And I learnt that such drugs are prescription drugs and could be habit forming drugs. I decided not to take any drugs again unless I get somebody who will attend to me very well. Doctor, could this be a psychiatric or psychological case? What do you suggest that I should do? Could you please direct me to a specialist who could handle this problem well? This problem is seriously making my life very, very uncomfortable. It seems like I easily get angry. That is unlike me. I am running out of patience; please help me. Ask to see a psychiatrist. Be very plain with him. Tell all. He’ll help you immensely. A neurosis affects your moods; a psychosis affects your reality and personality - like your anger. I have a problem, since I started family planning I have not been seeing my periods again. I started the family planning in April, 2010, i.e the three months injection type. Since then my period stopped coming. Up till now I am still taking the injection.  Please advise me what to do if it is to continue or discontinue? What to do? Simple. First rest assured that sooner or later, your periods will return. That's for certain-- the earlier they'll return will depend on how early you decide to stop the offending shots. Second, go quarrel with whoever led you into the use of this method of Family Planning without adequately preparing and counseling you about what to expect. That's one thing everybody should understand--never use any methods of fertility control without asking full knowledge of the side-effects. Your complaints here are very well known for this 3 monthly injection method, and what we do is tell ladies who still want to have kids not to consider it precisely because, they may not see their periods at the exact moment they need it to get pregnant. Of course, if you are done with child-bearing it might even be a relief for you not to see periods for prolonged stretches of time. So, it boils down to this-- if you still want babies stop this method. If not, continue.

I am a very good reader of your column and a girl of 16[sixteen]. I must commend you for the way you tackle peoples' problems. Some things have been bothering me and I have a few questions to ask you about them. [1] Can a woman/girl have hernia? If so, what are the signs and symptoms? [2] What is the difference between a "labia minora" and "Bartholin's glands" in the external genitals of a vagina? [3] I read in a book that the labia minora varies in size, i.e one can be bigger than the other. Is it true or is it an infection? If it is an infection, what can be used to cure it or prevent it from getting to an uncontrollable state?

Dear Worried Girl,
Yes ladies do have hernias too---a case of what a man can do a woman can do also! The features are hardly different from that of the man---they're the same ---swelling in the groin that comes and goes especially with any form of strain like coughing, etc etc. Note that hernias are dangerous--very much so. Anyone who has or thinks he has a hernia is hereby advised to go, quickly and see a surgeon who will confirm or deny the suspicion and do the necessary. Hernias kill if mismanaged and allowed to strangulate--which is practically all the time if you do not allow your surgeon/doctor to help you with it. Now for the external genitals. These are also called the vulva-- collectively. The labia minora are different from Bartholin's gland. All women are blessed with wonderful external genitalia that are almost uncontrollably attractive to men--in fact they make a man crazy anytime he sees them or thinks of them. This consists of half a dozen structures: the clitoris is at the upper end of the oval shaped area called the vulva or external genitals. It is covered with a small hood which divides out into the two minor lips --one on each side of the oval, called labia minora--singular-- labium minus. The large lips or labia majora are thick fat filled structures that on both sides cover the minor lips -- you have to open the large lips to see the minor ones. The hymen grows around the introitus which is the entry into the vaginal barrel. In between the cltoris and the introitus is the urethral meatus -- the spot from which the lady urinates. The minor lips having bloomed in the middle of their length to accommodate the vagina now join together, once again, below the introitus, to form the vaginal vestibule. Now, the labia majora are the structures in which you find the Bartholin's glands and their own job is to provide lubricant secretions for sex. They cannot be felt or seen when they're not diseased --so you will not feel them unless you have an infection. The sizes of the minor lips of the vagina may vary but mostly inappreciably unless of course when there is disease. An infection will involve pain and swelling and loss of function-- do you have any of these? See a doctor for proper cure after correct assessment because the commonest cause of Bartholin's gland trouble is Gonorrhea.

Stroke and Witchcraft?

Stroke and Witchcraft?
 
Dear Doctor,
 
My brother suffer red a stroke about
a year ago. He was flown abroad and later sent back home. He has been getting
better but only slightly and a bit too slow for my liking. He is no longer in
the hospital because we have been made to realize that what he needs now is
some physiotherapy and so on. But I am not satisfied. He can hardly talk or recognize
any body. I am worried - some of our relatives are sure there is witchcraft
involved and, as if they are right, certain people at home, I am told, have in
fact, already confessed their involvement. Doctor, what can I do? What do you
think about the witchcraft angle? What can I do to help? I don't want to lose
him! Please help. Thank you

Dear Mrs Humuani,
 
Stroke is defined as "a sudden
loss of brain function caused by a blockage or rupture of a blood vessel to the
brain, characterized by loss of muscular control, reduction or loss of
sensation or consciousness, dizziness, slurred speech, or other symptoms that
vary with the extent and severity of the damage to the brain. Also called
cerebral accident, cerebrovascular accident and apoplexy, stroke involves a
sudden loss of consciousness resulting when the rupture or occlusion of a blood
vessel leads to oxygen lack in the brain. Hypertension is usually the disease
behind a stroke. It increases the pressure --or force of flow-- of the the
blood vessels to the brain are carrying beyond the point of endurance and the
poor vessels burst --rather like when the water pipe from water corporation
bursts at some point before it gets to your house! Then you will suffer a
"no water" situation-- just like the brain suffers a no oxygen
situation causing brain cells to die. And since nerve cells do not regenerate
when they die, rehabilitation will have to take long -- the function of dead
nerve cells will need to be taken over and learnt slowly (by dint of repetition
and practice) by other nerve cells unaccustomed to those functions. When there
is a blockage of blood vessel rather than a busted blood vessel at the root of
stroke however, then we have -- not hypertension -- but something else as the
underlying disease condition causing the stroke. A good example is when there
is a Road traffic accident and some long bones are broken-- maybe the thigh
bone or femur. Here the displaced bone marrow fat from the broken bone may
travel through blood vessels as an embolus to go and block vessels in the brain
and cause a stroke. Emboli may also form from the cholesterol plaques found in
the inside walls of old and ageing atherosclerotic blood vessels --again dislodging
and travelling to block smaller blood vessels in the brain to cause stroke.
This is why a high cholesterol level and atherosclerosis are dangerous. The
care of a stroke patient involves making sure they are receiving as and when
due their medications against hypertension (commonest cause in these parts) and
or whatever else may be the root of their stroke, because if care is not taken
the high blood pressure will soon cause another stroke in a patient that has
just been diagnosed of the same!!! Psychologically, there must be committed,
caring , supportive, non- complaining and anticipatory care best given from
loving members of the family. You see, a person with stroke is either paretic
or paralyzed --(very weak or totally non-functioning muscles) and cannot move
by themselves to some or every degree and may not even be able to help
themselves to toilet. So as not to demoralize them and thereby make them prone
to further deterioration, the care and support must be of high quality and
committed -- it takes very little carelessness for a paralyzed patient to
develop a bed sore -- deadly in its effect - and usual when nursing care is
deficient -- a paralyzed patient's position must be changed at least every 2
hours!! Spiritually, you will find that prayers will help --seriously. You
can't pray? Well psalms are ready -made prayers and work if prayed
conscientiously. Witchcraft? Confessions? Hmmh. Let's just be open to the
suggestion  that many diseases
disabilities, discomforts, disasters and depressing adversities could be
negatively spiritually engineered by agents of darkness. If you read my book ,"Destroying
