Rashes, Itching, Fast Ejaculation

Good Evening Doc, I am 20yrs of age and I had a small rash d@ made a portion of my penis cap to look lighter in colour) 4yrs ago and it has been increasing around my penis cap till now. I also ejaculate (very little though) sometime while stooling and still have wet dreams. My penis cap has become so sensitive that I don’t last long while having sex before I ejaculate. @times I have little swells in form of boils on my pubic hair area and with time, they disappear again. Whenever I scratch on them, it always feels good though. Doctor please what can I do! I am so shy 2even go 2 a hospital.
