Dear Doctor,
I have a problem with my right breast. I started feeling pain in it about three weeks ago so I went to a general hospital close to my school. A doctor tested me and told me I had a lump in my right breast but  that I shouldn't panic that it could be removed anytime. He placed me on some drugs--ampliclox; folic acid and I've forgotten the rest. I just want your advice on what I should do because I'm still in school. I would be very grateful sir if you could reply through my mail

Dear Ladi,

Apart from malignancies or cancer, the breasts may be afflicted with bacterial infection called mastitis. Antibiotics, like the doctor gave you will treat that condition. Apart from mastitis common among breast feeding women, other non-cancer, non life-threatening or benign breast conditions are many and  they can cause symptoms, and some are linked with a higher risk of later developing breast cancer. The more notable among them include: Adenosis, Simple Breast cysts and fibrosis, Ductal and lobular hyperplasia, Fibroadenomas and Fat necrosis and oil cysts. It is the doctor's job, when you go to see her to check you out and do the necessary to determine what really your affliction is and how to treat it. Your own duty is to check your breasts regularly; not wait for when you feel pain or have trouble, but actively  and REGULARLY examine and re-examine them BY YOURSELF. This is called Breast Self -Examination and for how exactly to do this, I have resorted to  www.Breastcancer.org - Breast Cancer Information and Awareness for much of what follows in bold italics -- 

The Five Steps of a Breast Self-ExamStep 1: Begin by looking at your breasts in the mirror (as you face the mirror) with your shoulders straight and your arms on your hips. Here's what you should look for: i)Breasts that are their usual size, shape, and color. ii) Breasts that are evenly shaped without visible distortion or swelling. If you see  or feel any of the following changes, bring them to your doctor's attention: Dimpling, itching, puckering, or bulging of the skin/A nipple that has changed position or an inverted nipple (pushed inward instead of sticking out)/Redness, soreness, rash, or swelling. Step 2: Now, raise your arms and look for the same changes. Again, stand sideways to the mirror and look for any changes as you look at your breasts sideways. Step 3: While you're at the mirror, look for any signs of fluid coming out of one or both nipples (this could be a watery, milky, or yellow fluid or blood).

Step 4: Next, feel your breasts while lying down, using your right hand to feel your left breast and then your left hand to feel your right breast. Use a firm, smooth touch with the first few finger pads of your hand, keeping the fingers flat and together. Use a circular motion, about the size of a quarter. Cover the entire breast from top to bottom, side to side — from your collarbone to the top of your abdomen, and from your armpit to your cleavage. Follow a pattern to be sure that you cover the whole breast. You can begin at the nipple, moving in larger and larger circles until you reach the outer edge of the breast. You can also move your fingers up and down vertically, in rows, as  if  you were mowing a lawn. This up-and-down approach seems to work best for most women. Be sure to feel all the tissue from the front to the back of your breasts: for the skin and tissue just beneath, use light pressure; use medium pressure for tissue in the middle of your breasts while you are standing or sitting. Many women find that the easiest way to feel their breasts; use firm pressure for the deep tissue in the back. When you've reached the deep tissue, you should be able to feel down to your ribcage. Step 5: Finally, feel your breasts is when their skin is wet and slippery, so they like to do this step in the shower. Cover your entire breast, using the same hand movements described in step. 


It is  important to note that women's breasts can feel lumpy, swollen, painful and discomforting a few days before your menses begins  to flow .Such feelings may usually go away after the period has ended. Again, if you are an adolescent, you a retaking oral contraceptives , or are on other hormonal therapy or you are pregnant, your breasts may feel generally funny, lumpy or painful. However, when lumps or pain persist throughout  your menstrual cycle and not just during your periods, pain in your arm-pits , then you should be more sensitive about seeing a doctor. To help  decide if you are normal or not, try to keep a  diary or journal for 2 or 3 months noticing and recording how your breasts feel so that you can spot changes either related to the menstrual cycle or not. Also, just before your periods, you can wear a larger brassiere or a support, soft or sports bra so that  you feel more comfortable. Finally, just after your periods, once a month, is the best time to examine your breasts and if you have stopped having periods, take a day you can easily remember--maybe first of the month.

 Dear Dennis,
If you feel this pain principally when you go to stool and you do not bleed, then we can safely assume that perhaps your stools are hard--in other words you get constipation and that it is this constipated, hard stools that are the cause of the aching anus. You must understand that hard stools are the leading cause of a dozen or so conditions that can ache your anus; especially if associated with stooling. Other conditions are a)Cuts in the skin or tissues(we call these fissure-in-ano) of the anus --again; dry hard stools cause this, leading to b)infection and maybe c)a boil or d)an abscess. This may end up as  d)a "fistula-in-ano" when the pus from the abscess causes a track in the tissues as it tries to find its way out. e)Injury to the bum as well as e)Anal sex f)Pile or hemorrhoids especially when they are clotted and g)infected; h)STD's and proctalgia fugax,a self limiting condition common among teenagers as well as i)prolapse of the rectum; common among women who have delivered many babies -- ALL can also cause aching anus. It is only after you have seen a good doctor and he has listened to your full description of the problem, cross examined and physically looked you up can anyone say which of these causes you have. Before you see the doctor, these half a dozen measures will help you tremendously a)Sit on warm water at least twice a day --sitz bath it's called b) Drink lots of water--up to 10 glasses a day--2 each on waking and retiring and at each meal; c)Eat a good portion of one of these fruits/vegetables daily so as to get plenty of fibre into your gut which will automatically soften your stool--pawpaw, pineapple, garden eggs, water melon, cucumber, apples and golden melon-- d)Go to stool once nature calls--do not delay until it is unbearable; e)Get lots of exercise; f)-- and I call this "Dr Oluranti's Technical Knock Out"-- use some Vaseline or other lubricant on your finger to lubricate your anal canal before you stool and no matter how hard the stools, they will be easily and "greasily" slipped out of your system with great relief. Sir, I will be very happy if you will be able to answer and publish my questions this next week. Please I need the answer urgently also that my mates out there can learn. I am a regular reader of your column every Thursday in Daily Sun till I had a problem which I will want you to help me with.I am a girl of 20 years old who just removed a baby; it is two weeks now and since then I have been bleeding but now it is not as heavy as the very first week and now it´s not even reddish like blood, it´s just brownish colour.In fact, I stopped putting pad and just wear my pant; when I am wet then I will go and change because, I see it as wasting of pad since the thing is not much. Two days after the removal of the baby, my breasts became swollen and sometimes it will be paining me and by that time water will be dropping from my breasts. This continued till Sunday last week when it stopped and my breast now became too soft and had come back to the normal size. My question now is,are there some foods and drugs that I need to take? If yes, please prescribe them for me. Secondly, since that time I noticed that from my navel down to my waist /abdomen; even my waist is paining me. I forgot to tell you at the beginning that I  gave birth to the 3 months and 14 days old baby, in fact, I was placed on drip. I took about seven  (7) bottles of drip. It was the eight (8) one which was on the 4th day that the baby came out with the placenta. Please doctor, if there is some precautions I have to take now before it is too late; please tell me and advice me on other things to do.

Haba!!!You do not want to be removing babies at 20 years of age; do you? Babies only come when we have sex. Sex is best inside marriage. So, the age old advice remains, abstain if you do not want a baby;okay? But if you cannot abstain then,to be sure you do not get into unwanted pregnancy trouble again, you need to learn Fertility Awareness and Control. You will learn more about this at my blog www.mediamedix.blogspot.com.  If you cannot go to the blog and you want the gist here it is -- as a lady the length of your menstrual cycle (number of days) if divided by 3 (eg 28 days divided by 3) = 9.3 days. To get pregnant, have sex in the middle one-third days; to avoid pregnancy, never have sex in the 9 days in the middle one third. That's a rough rule but I will send my free report titled, "Fertility Awareness and Control" to anyone who asks for it by e mail. Now for your other concerns-- Every new mother bleeds for some time after birth and it is normal that the stuff coming out will first be blood red and later be less copious and change colour to brown. We call the stuff lochia. In the same manner, it is normal for your breasts to drip fluid and pain you -- after all if you had not removed the baby you are supposed to be breastfeeding the baby. It is the breast milk which your breast produced in response to the baby's presence that caused your breast discomfort. The pain in your pelvis (from navel down to waist) is best evaluated by a competent doctor/gynecologist. It is possible you contacted Pelvic Inflammatory Disease ( PID) in a mild form. It would need expert treatment so that it does not  block your tubes and make you unable to get pregnant in future. 8 "drips" show that the baby you removed was unripe to come out and you really had to force it to come out. As to the age of the baby, it is usually preferred that a baby be removed at an earlier age (than the 15 weeks in your baby's case) when the process of removal is much easier.
In summary, please learn Fertility and Awareness Control (you can send me an e mail requesting for it or just go to the blog and read it) and practice it. It will save you from unwanted pregnancy

Dear Doc,
I am the one that called you yesterday, I am a 33yr old man from Enugu State. I have a serious problem which is trying to ruin my life. I engaged in a relationship which resulted in disease and right now I am dying under some terrible pain. When I urinate I always pass some milky fluid. I have a swollen scrotum, I was diagnosed by a doctor who said that I  had a hernia. I went through surgical operation but to no avail. I have taken my problem to so many hospitals without any better result.
These are the symptoms: milky fluid coming out of my penis when I urinate, swollen scrotum (testis), weak sexual emotion, pain in the swollen scrotum (the right one). I suffer the pain most when I have a strong erection and sometimes migraine headache with it. My lady has loved and followed me and our relationship has resulted into marriage; and on the bed, presently I am now faced with these sexual problems--
1. I am unable to go more than one round , and release my load in a second.
2. After releasing I could not go again until the next day.
3.The most worrisome part  is that I cannot have or maintain a good and strong erection, my manhood always as soft as foam.
4. My early morning erection is no longer there.
5.During ejaculation my semen used to shoot but now it oozes out.
Please I am always ashamed of myself whenever she calls for sex. I am thinking of losing my life than live and seeing myself in this unbearable condition. My wife has tried to bear with me but right now, she is threatening to leave me if there is no solution to it. I also pity her for what she has being undergoing for so long.
I was subjected to a lab. test and below are the result. *Motility rate 10% Actively motile 30%. *Sperm count 5x1000000 cells. *Viscosity.. slightly viscous. *Pus cells... 1-3hpf ph 7.5. *Morphology..25% abnormal cells. The test also showed that I have a STAPH AUREUS infection. Please I still count on you for help. Sir, I want you introduce me to professional doctor in this area or the kind of drug that will eliminate the pain and restore back my potency. You have a Chronic Prostatitis --any male that discharges in the absence of sex has it--and your case shows how pathetic ignorance (of even medical practitioners) can be. Sexual activity tend to be vigorous, wild and unrestrained among young men. They catch a bug and infect their prostate -- an internal organ in the male that can be called a man's womb. Prostatitis sets in and can be hard to get rid of.
Final and lasting solution is available to you if you follow instructions for, though we are medical practitioners too we have seen where, why and how too many of our colleagues fail to successfully handle this very serious challenge to male health and well--being. Of course, there are complications already -- and that includes the swollen testicles -- it is a case of chronic epididymitis secondary to your prostatitis and the painful thing is that this ailment will be eating up your sperm count all this time -- you can see you haveonly 5 million when the normal is over 20 million sperm cells per ml. This is how low or zero sperm count develops, many times without the kind of warning bell your pain has served you. In other words, infection is eating away at a man's ability to be a real man or father a child and yet, since he has NO PAIN at all, he knows nothingbaoutit. That is Chronic Prostatitis for you. Again, your case shows very clearly the link between three very seemingly unrelated concepts or phrases
a)Sexual inadequacy(SXI) or erectile dysfunction
b)Chronic Prostatitis(CPR) and
c)Staphylococcus aureus
In a nutshell, understand that Staph --or any other germ (especially E.Coli, Kelbsiella, Proteus; Streptococcus etc --  some of them STD's and, some not so) can invade a male's system, infect his prostate; give him a CPR and then cause SXI secondary to the CPR. In my experience, CPR --hidden, unsuspected, undiagnosed (by SFA--seminal fluid analysis with m,c,s-- is the cause of up to 85% --if not even more of SXI). SXI is curable; CPR too (but by experienced doctors). My Complaint About African Doctors

Dear Dr Bibbi,
Your write-up on the subject of Organ in Bondage/Sexual Inadequacy in the Daily Sun recently, refers. It made an interesting reading.
I am worried that some Africans Medical Doctors do not take the spiritual aspect of ill health into consideration while managing cases. As a result when you make your complains they tell you that it is psychological especially when sexual problems are concerned. It is sad indeed. I have a similar problem to that of the writer of  that piece. I got married in Dec. 2013 and since then it has been one form of spiritual attack and the other. Oppressed by witches while sleeping, finger prints on my body, multiple sex dreams with unknown partners, had two fatal motor accidents, and now I find it difficult making love to my wife because my organ will not rise or become erect!!! Since we got married we have believed God for the fruit of the womb but surprisingly I find it most difficult to make love to her during her ovulation.
Once I or my wife have this sex dream it becomes  difficult for me to get an erection. The size of my organ becomes  so weak and very smallish. Just as written by the complainant in his letter I have to beg and rub my organ before I can manage a weak erection sometimes. All these are the opposite of my former self before I got married. 
 My type of job take me away from home for a week and gets me off duty for a week. If I am alone in my station I WAKE UP almost every night to see my organ strongly erected usually about 1.30am -3.20am.
I am a born again Christian so all we have being  doing is to get into stubborn and aggressive prayers. But the more we pray the more the problem gets tougher. The joy that greeted our marriage is almost disappearing.We  are weeping, bruised and heartbroken.
 We consulted a Medical Dr (Gynaecologist) who put me on PROVIRON, to solve the problem and, yes, the first three days of the ingestion of the drugs it worked, but  thereafter it was as if the drug was working the opposite of what it was supposed to do. (I have taken the drug for about 3months now.) How can orthodox medicine explain that?  I must tell you that some persons ( my former girlfriend who is Genotype AS  same with me, as well as the guy my wife had relationship before I met her) put obstacles on our ways to stop our wedding but God saw us through. They even threatened that if I married  my wife that our marriage will not last.  Certainly out of ignorance I did not take their threats serious, but now I know better. 
 Doctor, it has been a difficult time for me and my wife. Our problem is no doubt purely spiritual and we have taken it like that. We have taken our case to the supreme court of God Almighty. We believe God is able  to deliver us and give us victory in Jesus Name.  On this basis, it is painful when our fellow Africans
 Doctors do not accept that some ill health are spiritual; rather they will say that such sexual problem is the state of the mind. Those finger prints on the body on waking up from sleep, those strong erections in the midnight when alone, etc are they also psychological?

Let me help you by making some six pertinent points. 1)Doctors, African or no;t are doctors, not priests, pastors, clerics or shamans. They do not have any training or exposure in school to the things you are talking about, so, they cannot and do not handle such matters. 2)Doctors take training in physical sciences and psychological sciences; full stop. Nobody teaches them about he spiritual and it is not the fault of anyone. 3)The scientific basis or way of thinking that is the root of their training does not recognize anything THAT CANNOT BE SEEN or PROVEN PHYSICALLY. 4)You will agree that the kind of understanding you say is lacking is more available in church or seminary, Bible, Koranic or spiritual school. 5)In point of fact, because the average African understands these things without any doubt, African doctors tend to be more understanding than their Western counterparts in these cases. 6)For you to get not only a sympathetic hearing, but help to get rid of these your troubles, you need the rare professional who combines an understanding and proficiency in these two worlds that seem anti-thetical to each other-- the physical and the spiritual. Dear Doctor,
I happened to come across your column at a friend's place with whom I am staying presently and I want to say I like the way you advice and re-assure those who write you with their problems.This has encouraged me to write to you. I would like to know if it is possible for someone like me whose parents are not diabetic to have diabetes .Is it possible for a young boy like me who is twenty-one years to have diabetes just like that? What are the symptoms of diabetes? Does it make the male organ small and weak? Why I am asking this is because sometime ago I noticed that after I urinate, some whitish liquid comes out and it draws like okro though it's not quite as thick. It is clear and whitish and doesn't smell badly or cause pain . I thought I had STD at first but I was surprised because I have never slept with any woman before and I don’t masturbate. Last month I summoned courage and went to the hospital and to my greatest surprise I was told that it was semen escaping from my reproductive organs.Now one of my friends has told me that this is a sign of diabetes. Is it true? I am afraid because I know I take a lot of sweet things. Another one of my friends told me that this condition is caused by smallness of my organ. He further advised me to start having sex otherwise it would start growing smaller. Dear doctor, how do I stop this problem? It has been embarrassing me and it sometimes stains my trousers . Do I go for a diabetes test? Likewise is it really true that lack of sex on reaching maturity causes the male organ to remain small? How do I enlarge my organ? Is it through starting to have sex frequently?I thought of going to one of these places advertised frequently but I was held back because I know that some of them are quacks and I don’t know if such enlargement would leave me impotent .

Diabetes does not of necessity pass from parents to children-- its inheritance pattern is not so direct--that is even when parents have the disease. Yes diabetes runs in families but there are environmental determinants that can trigger its manifestation even when the genetic disposition is not there. So diabetes can just arise out of "nowhere"--yes. Diabetes does not the cause any of the things you have(although it can cause low libido--disinterest in sex as well as erectile dysfunction or impotence). You say you've not had sex before but what about necking and other such things including genital or oral contact? They are significant in these cases, you know. Do your test but let a doctor order it and it should prove that you don’t have diabetes. Diabetes is not always inherited. It can arise from nowhere. It's major symptoms are frequency of urination, overmuch thirst, tingling sensations in the feet and hands, frequent infections like boils, furuncles and carbuncles, slow healing ulcers, or cuts or wounds, blurred vision, always tired or just no symptom at all.For those with no symptom like you but who want to find out if they could have or be prone to the disease the laboratory may find they have what we call an impaired glucose tolerance when and if they do the test of their glucose tolerance. This your okro like discharge is a sign; not of diabetes but of chronic prostatitis –a disease that is usually a complication; not of an STD(it can be) but of UTI (urinary tract infection) which any one can have whether they sex or not. The genitals, you see, perform three functions: recreation, urination and procreation (reproduction). Now, chronic prostatitis is a very insidious disease and because the prostate is an organ through which all these three functions of the genitals pass, it could cause problems in all the three-- that is the cause of your problem and not diabetes. Anyone who, as a male, discharges any stuff without sexual arousal has chronic prostatitis-full stop. Most young men your age feel that they must have a horse size organ to be men. Not so. It is not quantity in this matter but quality that women consciously or unconsciously look for. If you have a king size organ and lack technique your sex would not be able to satisfy a woman-- never forget that.Also never forget that it is indiscriminate sex that causes problems(HIV,STD,AIDS ), not diabetes. Again, remember that diabetes does not cause these problems that you ask about. Your friend will push you into indiscriminate sex and that's where the trouble will come from; Okay? So beware.Small organs are not the cause of discharge, diabetes or anything; Okay? Don't buy that notion.It is un-educated; illiterate. Full stop.Diabetes is a disease caused by the body's inability to handle sugar /carbohydrates properly because insulin is deficient or not working. It does not mean that sweet things cause diabetes--else ask the people of Western cultures who would have been suffering an epidemic of diabetes on account of their liking for sweets at almost every meal--dessert, iced cream, jelly, syrup etc. A correct, balanced and nutritious diet as well as exercise and weight control are the things that help prevent diabetes.

I'm a 24yr old guy who is indulged in self-discovery and who is looking to self-actualization and self-fulfillment. How glad I am this day, sharing with you a problem that has -for some years now- been weighing me down. a problem that instills in me the feeling of faltering while partaking in an activity, of not doing what I ought to do, and the unwarranted feeling of scrutiny and social insecurity whenever I'm in a public place. it is a secret enigma that I've been living with -hiding it within the depths of my mind- for far too long. Pouring out my gravely over burdened heart to one whom I feel would understand and bear with me -and possibly help alleviate or eradicate- the ceaseless pains, sorrows and worries which I feel deep inside, I believe, is one great step I'd be taking toward recovery from my burdens. I've always taken it to be a personal affair; if it wasn't a problem I see as being so humiliating, I'd have referred to it as "my dear." This is the very first time I'd discussing -with anybody, not even my parents or siblings - the awful experiences in my social life. Quite strange and incredible, isn't it? Well, you'd better believe my story because I'm telling you the truth and nothing short of it. But..."

People with social phobia fear that their behavior will lead to negative judgment by others, even though they recognize this fear is illogical.
